Cardiac evaluation in adults with dengue virus infection by serial echocardiography

Abstract Background Dengue virus infection (DVI) is a major health problem in many parts of the world. Its manifestations range from asymptomatic infections to severe disease. Although cardiac involvement has been reported DVI, its incidence not yet well established. Methods From July 2016 January 2018, patients hospitalized at Hospital for Tropical Diseases, Faculty Medicine, Mahidol University, Thailand, with dengue confirmed by positive NS1 or immunoglobulin M findings, participated study. We characterized and change function serial echocardiography levels troponin-T creatine kinase-myocardial band (CK-MB) on day admission, defervescence, first hypotension (if any), 2 week follow-up. Results Of 81 evaluated, 6 (7.41%) exhibited elevated biomarker levels. There was no difference clinical presentation amongst fever, haemorrhagic fever (DHF) shock syndrome (DSS), except amount bleeding. Cardiac found 22.2% patients: 3 (3.70%) had left ventricular systolic dysfunction, transient diastolic increased least one (troponin-T CK-MB), small pericardial effusion. Myocarditis suspected only two (with DHF); thus, myocarditis uncommon infection. Three developed DSS during admission were transferred intensive care unit. Conclusion adults common, ranging myocarditis. Abnormalities resolved spontaneously follow-up, without specific treatment. that DHF significant risk factor involvement. Echocardiography investigation choice evaluating haemodynamic status especially dengue.
